Follow our step-by-step procedure to claim your prize.

1.  Take one of the waivers that are provided in your basket (or print from below), along with a voucher.

2.  Fill out the waiver and place it back in the basket by Wednesday December 7th at 3:00pm.  You are not committing to anything.  this is just our procedure to gather contact info and make starting the program hassle free.

3.  We will be picking up the waivers at the specified date listed above.

4.  After we receive the completed waivers, we will be giving you a call on the following day.

5.  We will set up everything for you.  After our initial contact on the 7th, your vouchers will be activated and ready to use.

6.  The next step will be coming into our facility for an orientation and screening process to ensure this program will be a safe and effective program suitable to your needs.  We will set up an appointment during our initial phone contact.

7.  Finally... Show up and get ready to work!  Remember, we will be picking up the waivers on Wednesday, December 7th.  Don't miss your opportunity!
